I, the owner or authorized agent for the owner of the above pet, have been informed of the following conditions and considerations related to international pet travel.
Countries throughout the world have their own specific requirements and regulations related to pet travel. Heart of Chelsea does its best to ensure such requirements and regulations are up to date to forward accurate information to me in order to be compliant with my target destination’s protocol(s).
I understand that because of the dynamic nature of these protocols, they may change without prior notice. I have been made aware that there is no alert system in place to assist veterinarians with these changes should they occur.
I have read, understand and am in compliance (according to the most recent information Heart of Chelsea has) with the protocol for my pet to travel with me. I also understand that this may not be the most current information, and have been instructed to contact the consulate or embassy of my target destination to verify these requirements as factual and current. I hereby assume responsibility for all travel arrangements for my pet, including verification of the correct requirements for the veterinarian to follow.
I understand that Home Again microchips are not guaranteed to work in all foreign countries and if I am concerned about microchip compatibility, I have been instructed to call the microchip manufacturer Home Again at (888) 466-3242 for information pertaining to further options or instructions that may be necessary.
I have read and understand the potential complications associated with international pet travel. I understand Heart of Chelsea is not responsible if the international travel paperwork is not endorsed due to USDA office closure/turnaround time and/or FedEx delays. I also understand Heart of Chelsea is not responsible for the cost of the international travel paperwork if it cannot be endorsed by the USDA or my flight is cancelled or delayed. Should further action be necessary at my port of entry, I accept full financial responsibility and release the staff at Heart of Chelsea Veterinary Group of all responsibility and liability.
